Dear Members of PHUMC:
The Long-Range Planning Committee is responsible for formulating a strategic plan for the future of Pulaski Heights United Methodist Church and we need your help. The next few years will bring many changes and opportunities for PHUMC. This year, Phase III of our building project will be up for consideration and. in 2012. our Church will celebrate its 100th anniversary. We are asking for your help in preparing for our next centennial.
Please take a few minutes to complete this two-part survey. Print it and complete it and return it to church. Part I asks for your comments, concerns and suggestions which will help the committee focus on things that are important to you – the congregation. Part II asks for your evaluation of specific church programs and ministries. Your input will be incorporated into the long-range plan for the church. Please return this form to the designated drop-box in the church office, by mail or on-line by Sunday, May 25. EVERY member of the church is encouraged to participate and you are free to make copies of this survey for other household members.
